Identification Policy

All patients must present a valid, non-expired state issued Driver's License, state issued ID card, US Passport or Passport ID card.

 

Cancellation Policy

We know your life is busy and we value your time. With that in mind, our office does not double-book patients and requires 24 hours business notice to cancel or change an appointment.  If proper notice is not provided, a no-show fee of $50 will be charged.

SMS Consent Communication:


Information: Phone numbers obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.

Types of SMS Communications: If consent has been given to receive text messages from LIGHT Hearted Wellness Care, LLC, messages may be received related to the following: Appointment reminders, office updates, billing notices, limited follow-up, and medication management questions (HIPAA compliant)

Message Frequency: Message frequency may vary depending on the type of communication. For example, you may receive 2-3 text message per week for appointment reminders, practice updates, billing notices, follow-up communication, and limited medication or treatment questions in accordance with HIPAA guidelines

Potential Fees for SMS Messaging: Standard message and data rates may apply, depending on the carrier's pricing plan. These fees may vary if the message is sent domestically or internationally.

Opt-In Method: You can opt in to receive SMS messages from LIGHT Hearted Wellness Care, LLC either verbally and or through consent forms.

Opt-Out Method: Opting out of receiving SMS messages can be done at any time by replying "STOP" to any SMS message received. Alternatively, direct contact can be made to request removal from the messaging list.

Help: For any issues, reply with the keyword HELP. Alternatively, help can be obtained directly from us at https://www.lightheartedwellcare.com or calling the office. If SMS messages are not desired, please make the practice aware and you will be opted out.

Prescription Policy

Our office provides refills and renews prescriptions only during appointments. This practice reduces prescription errors, improves patient safety and encourages appropriate follow-up. Patients will receive enough medication or refills to last until their next recommended follow-up. Patients are asked to track their supply, and to ensure they have an appointment scheduled before they run out of medication or run out of existing refills. Please be proactive in your care and track how much medication you have and how many refills remain on the prescription, and ensure you have an appointment to see the doctor before you're out of medication. 

 

Lab Work Request Policy

To ensure patient safety and quality, lab work must be completed prior to follow up appointment. If available, our office will work with your Primary Care Provider (PCP) to obtain your current lab work. This will aid in our coordination of care efforts and reduce duplicative lab orders.

Insurance

We accept most insurance plans including Medicaid, Medicare, Cigna, Aetna, Blue Cross Blue Shield, to name a few. Our staff will help you with insurance questions and scheduling. 

Rates Without Insurance

For patients without insurance, we offer a prompt pay discount. Medical expenses are tax deductible. Our cash price is $200 for a new evaluation and $150 for follow ups.
